To schedule an appointment, please include the following information in the Comments section:

1. Indicate your preferred date and time for your appointment.
2. Provide your primary/referring physician’s name and his/her phone number, if applicable.
3. Indicate your reason(s) for the visit from the dropdown arrow on the request form.
4. Add any additional information about yourself that you want Dr. Fan to know.

We will call or email you to confirm your appointment.

Once your appointment is confirmed, please visit our Patient Forms page. Print the forms and have the completed forms with you for your scheduled appointment.
